Role in Care Plans

Patients

• Describes symptoms and functional impairment
• Brings in documentation – past assessments, school reports
• Presents childhood history
• Asks family/friends to give collateral history
• Take medication as prescribed & report side effects 
• Engages in therapy and follows through on homework

GP

• Primary role in doing basic assessment and screening of symptoms and referring for further evaluation with psychiatrist 
• Engages patient in treatment
• Provides basic psychoeducation on diagnosis and focused psychological therapies
• Prescribes and monitors medications

Mental Health Professionals

• Develop a more complete understanding of functional impairment and treatment goals
• Help patient clarify treatment goals
• Delivers CBT and/or other therapy modalities
• Liaises with GP & psychiatrist

Psychiatrist

• Helps make sense of symptoms and functional impairment 
• Rule out differentials 
• Confirms diagnosis against formal criteria
• Recommends treatment considering comorbidities 
• Review treatment goals with patient
• Gives medication recommendations and prescribing details
